Why The AIDS Epidemic Matters To Black Lives

About a year ago I attended a gathering of activists who focus on issues for Black men related to mass incarceration, the war on drugs, police brutality, and stop-and-frisk issues, shape notions of masculinity for Black men and boys. I was asked on the last day to jump in and talk specifically about how homophobic violence and notions of masculinity impact Black men, particularly gay and bisexual men, and the alarming rates of HIV infection. After I discussed these issues, one attendee, a young straight Black man seemed annoyed that I was bringing HIV into the discussion, and offered, "We already know HIV is genocide against the Black community, and I wouldn't be surprised if it was man made."This isn't the first time I've gotten this response. Like HIV, many Black people view the policing and prison institutions as being intentional in their efforts to commit violence against people of African descent, and to destroy communities. Yet, unlike police and prisons, understanding government neglect or malfeasance has not helped inspire massive mobilizations in Black communities to challenge local, state or federal governments towards efforts to end the epidemic. In fact, it seems to have had the opposite response -- silence and resignation. But if we are to make the world make good on the mandate that Black lives matter, we have to be able to imagine and make good on Black led mobilizations toward issues including but beyond police violence, and HIV/AIDS provides us with some prime opportunities.Despite representing only 14% of the US population, Black Americans accounted for nearly half of all new HIV infections among adults and adolescents in 2010, a new infection rate 8 times higher than that of white Americans, according to the Centers for Disease Control and Prevention. The US Southern states, where the vast majority of Black people live, is also increasingly becoming the epicenter of the US HIV epidemic, with 7 of 10 US cities with the highest rates of HIV infections being Southern cities.In Atlanta, Georgia, a city known to have large numbers of Black gay residents, anEmory University study in March 2014 found the rate of HIV incidence in young Black gay men in Atlanta, Georgia at 12.1% a year. This rate is one of the highest figures ever recorded in a population of a resource-rich nation, and means that a young, Black gay man sexually active at 16-years-old is 60% likely to acquire HIV by the age of 30. In attempting to understand factors contributing to the high incidence rate, the study's researchers found a lack of health insurance coverage, unemployment, and incarceration as considerable social determinants of disparity among Black gay men.Black transgender people are affected by HIV in devastating numbers. In the largest survey ever conducted of transgender people in the US, the 2011 National Transgender Discrimination Surveyreports that20.23% of survey respondents reported to be HIV-positive and 10% were unaware of their status. This compares to 2.64% of transgender respondents of all races and 2.4% for the general Black population in the US.Though recent reports show new infections among Black women in decline, Black women still accounted for nearly two-thirds (64%) of all estimated new HIV infections among women in 2010 -- an incidence rate 20 times higher than that for white women.

Most notably, as numerous research studies demonstrate that Blacks are less likely to engage in risky behavior compared to their white counterparts, issues of racism are often cited as contributing factors to the HIV epidemic upon communities of color. In attempting to understand factors contributing to the high incidence rate among young Black gay men, researchers found that a lack of insurance, unemployment, and incarceration were drivers of the epidemic. Black transgender women face a lack of access to employment opportunities, which may lead to poverty and unstable housing, disproportionate policing and criminalization and a lack of access to health care.At the same time, access to care remains elusive for many African Americans, regardless of HIV status. A report by Kaiser Family Foundation showed that due to the blocking of Medicaid expansion under the Affordable Care Act in states with large Black populations has created a dynamic where 40 percent of Black people who would have gained health care through the ACA state Medicaid expansion are still without access. African Americans have worse outcomes on the HIV continuum of care, including lower rates of linkage to care, retention in care, being prescribed HIV treatment, and viral suppression.While rumors of HIV conspiracies are frustrating, I am not blaming Black people for being having them. A study published last month in the American Journal of Public Health, looking at the impact of HIV conspiracy theories in Black communities noted "Rumors may encourage behaviors that appear ignorant, but the source of conspiracy theory rumors about the origins of HIV/AIDS within the African American community...is not ignorance, but distrust combined with high social anxiety...The sources of the trust gap may be connected to the lack of sustained and effective funding for and attention to the general health of African Americans, the history of health professions' abuses against African Americans, and the persistence of HIV/ AIDS among African Americans (as the focus group data suggest, some African Americans see the very mention of high infection rates among African Americans as 'blaming,' which reinforces the sense of distrust)."So the question for me is, why are we so able to mobilize Black communities around policing issues, armed with historical fact and current events, and contemporary public policy challenges in policing and massive imprisonment, and yet the very mention of HIV disparities is reduced to blame?So whether you believe in HIV conspiracies or not, clearly we have an epidemic impacting Black people for which there is much improved treatment that undoubtedly saves lives but access to health care remains a huge problem, no less important than police violence. And yet when I mention HIV/AIDS in many spaces with activists, either I'm greeting with the issue of it being "man made" as a way to shut down the discussion, or the eyes sort of glaze over. Sometimes people who are very vocal on other issues as writers, scholars and activists, will only speak to me about HIV in private, thanking me for raising the issue because they lost a child, an uncle, a cousin, or many friends over the last 35 years to the disease. These losses, no less painful than losing a loved one at the hands of law enforcement, do not motivate many of these folks to get active in many of the fights around HIV prevention, treatment and care, that a dedicated few of us have diligently committed ourselves to--and have definitely included issues of policing/imprisonment, gender-based violence, education and employment as impacting factors. Yet we receive very little in the way of similar connections being made.Our Black future will require a focus on the varying theories, rhetoric, and institutions that perpetuate violence and premature death, whether directly connected to prison or policing.We'll know all Black lives will matter when all forms of Black suffering, including those from HIV and AIDS, aren't hidden in shame and silence.

A dying veteran was worried that his family would have to pay a $61,000 medical bill once he passed on, but the hospital decided that they would pay off his debts for him.

79-year-old Canadian military veteran Jim Holt was shocked when he found out that his family would not have to bear the burden of paying off debt that his insurance wouldn’t cover. Holt’s daughter Caroline told The Ottawa Citizen that she received the news while live on the radio talking about the situation her father found himself in with his insurance company.

“My father was in tears,” Caroline recalled. “He said, ‘Somebody finally listened.’”

Reports say that Holt returned to Canada after living in Italy and Argentina for 12 years because he found out he had Stage 4 cancer. While overseas, Holt suffered a broken vertebrae, which was caused by the cancer, so he decided to return to Canada to receive treatment at the Ottawa Hospital.

Upon being admitted to the hospital for treatment, however, the 79-year-old was informed that he had to apply for Ontario Health Insurance because he lived in foreign countries for so long. The health insurance has a three-month eligibility wait period, however, and because he was uninsured for those months, the military veteran was hit with a $61,000 bill.

Thankfully, the hospital decided to work with the insurance company and make sure that the man’s bills were paid, allowing him to now rest easy knowing that his family won’t have to worry about the bill.

“The hospital recognizes the financial challenges faced by uninsured patients who are not covered under a provincial or federal health plan and require non-urgent medical care,” Nathalie Cadieux, the hospital’s vice president of finance, wrote in an email to Holt’s daughter Caroline. “We review such cases on compassionate grounds and try to alleviate unmanageable financial burdens on families as a result of receiving necessary medical care following an illness or injury.”

First Federal of Bucks County recently presented generous contributions totaling more than $50,000 to nonprofit organizations that improve the lives of Bucks County residents. This gift is in addition to the funds previously distributed in 2014.

First Federal commits up to 5 percent of its net income each year to charitable institutions that serve Bucks County. For 2014, the bank was able to contribute an additional $50,000 to the groups it supports. That gift was shared among Aldie Counseling Center, Bucks County Opportunity Council, Family Service Association, Habitat for Humanity of Bucks County, HealthLink Medical Center, Network of Victim Assistance (NOVA), United Way of Bucks County, and Vita Education Services.

First Federal President and CEO Jeane M. Coyle and William L. Larkin, Chairman of the First Federal of Bucks County Board of Directors, recently presented checks for these donations to representatives of the organizations. The funds will assist with their day to day operations along with special events and programs.·     Aldie Counseling Center will use $5,000 to target the heroin problem in Bucks County.
·     Bucks County Opportunity Council will use $5,000 toward their Emergency Services support program, which provides financial assistance for families to overcome temporary barriers to self-sufficiency.
·     Family Service Association will use $5,000 for a program called Autism Awareness, Community Education and Support (AACES) to help adults with autism learn job skills.
·     Habitat for Humanity will use $5,000 toward Brush with Kindness, a program that helps low-to-moderate-income seniors stay in their homes by providing repair and maintenance work.
·     HealthLink Medical Center will use $10,000 to provide dental to low-income individuals who are working but have no health insurance.
·     NOVA will use $5,000 toward its Senior Outreach program, which provides outreach and education such as crime prevention information and safety planning for older adults.
·     United Way of Bucks County will use $5,000 toward Bucks Knocks Out Hunger, a community-driven project that unites donors, advocates, and volunteers to fundraise and then pack meals, including locally grown produce, lean proteins, and other items which can be hard for food pantries to secure.
·     Vita Education Services will use $10,000 to provide language and job skill training to low-income immigrants.

“First Federal of Bucks County is always the gold standard in community partners. It’s wonderful for us because not only do they run an amazing workplace campaign, but they are also big supporters of other projects throughout the year, including Bucks Knocks out Hunger,” said Marissa Christie, Chairman of the Board of Directors for United Way of Bucks County. “They were our biggest supporter last year and they are continuing to support that project this year and this means a lot to us because fighting hunger and food and security in Bucks County is something we care deeply about and so do our partners at First Federal.”

Tamara Hill works as a child and adolescent therapist in a fast-paced, residential and clinical setting. There's a lot of paper involved -- copies, printing, notes, documentation for calls to health insurance providers and worksheets for her clients.

"My entire office is full of paper, and many days I feel completely trapped by paper," she says. "However, as a therapist who is constantly in a vulnerable position to be sued, paper forms make me feel more secure when signed and filed. Anything can happen in an electronic system and paperwork is often lost or hard to get to."

Yet companies are touting the benefits of going paperless. Swingline, an office products company, recently released a survey on the risks of having paper documents in the workplace. The study found that 89 percent of employers use paper for record-keeping. Although 66 percent knew that shredding documents is important to protecting sensitive business information, 26 percent admitted they had thrown them out without shredding them.

When documents have been printed out, scanning them into the computer system can save money, according to Christopher Zybert, spokesman for NEdocs, who says, "The average document is copied 11 [to] 19 times throughout its lifetime, immediately increasing the cost associated with each printed page." In addition, he said, "The ability to electronically store, access, share and track files greatly reduces the time employees spend filing, searching for, copying, and/or mailing physical documents."

"Imagine that a company produces just 50 critical documents each day for filing," he added. "If it takes five minutes to file each document, you are paying for 250 minutes of filing each day, or roughly four hours. If you are paying your staff $15 an hour, you are looking at $60 a day, or $15,000 each year in filing costs alone. Additionally, being able to access and share these documents electronically enhances productivity and process efficiency."

Zybert also noted that office space used for paper storage will be freed up, saving more money. "A regular four-drawer file cabinet occupies about 15 square feet of space -- including the cabinet itself and the space needed to access it. If you are paying $40 [per square foot], one cabinet is costing you $600 a month."

"I no longer need to pay for paper, ink, filing cabinets, notebooks, staplers, printer/fax machines or space for storage," says Lark Ismail, a virtual assistant at Lark's Virtual Solutions, who's gone completely paperless.

Going paperless also allows Ismail to work more efficiently. "Now that it's all online, I can search for documents by their title or a keyword they contain, and I get the right one instantly," Ismail says. "I can also search for something specific in long documents and get to the right section instantly without having to scan through all of the pages." That leads to a doubly advantageous scenario. "In the long run, saving time saves me the most money," she says.

Here's How Health Insurers Dream of Replacing Obamacare

We got a glimpse last week of what would happen to our health care system if Republicans increase their control of Congress and win the White House in 2016.

Gone would be the part of Obamacare that Americans tell pollsters they don't like: the requirement that they enroll in some kind of health plan or pay a penalty that grows more severe every year. In addition, the GOP would get rid of the provision mandating that employers with more than 50 workers offer subsidized coverage.

But the GOP would also eliminate the existing parts of the law protecting us from insurance company practices that used to keep millions of us in the ranks of the uninsured and underinsured--and just an illness or accident away from financial ruin.

Of course the sponsors of the Republican alternative --called the Patient Choice, Affordability, Responsibility and Empowerment Act--or Patient CARE--don't spin it that way. In fact, the language they use makes their plan sound like a simple, common sense, no-brainer alternative to the Affordable Care Act (a.k.a., Obamacare.)

"We can lower costs and expand access to quality coverage and care by empowering individuals and their families to make their own health care decisions, rather than having the federal government make those decisions for them," said Sen. Richard M. Burr (R-North Carolina), one of the three authors of the plan. The others are Sen. Orrin Hatch, (R-Utah), who now chairs the Senate Finance Committee, and Fred Upton (R-Michigan.), the chairman of the House Energy and Commerce Committee Chairman.

As always, though, the devil is in the details. The reality is that the GOP plan would take us back to the days when insurers could sell junk policies, charge older folks more than they can today and calculate premiums based on a person's health status. This means that a breast cancer survivor or a diabetic or someone recovering from a heart attack--or even a young person born with a disability or congenital disease--would have to pay a fortune for decent coverage if, God forbid, they let their existing policy lapse for two months or longer.

The Republican sponsors say their plan would restore freedom of choice they claim was taken away by Obamacare. There is some truth to that. The Affordable Care Act requires health insurance policies to cover 10 essential benefits, ranging from preventive care to prescription drugs and a stay in the hospital. People no longer have the freedom to buy policies so skimpy they have to pay almost all of their medical bills out of their own pockets. The Patient CARE Act would restore that freedom. And it would also restore to insurance companies the freedom to set annual limits on coverage.

The health insurance exchanges would disappear in most if not all states because the federal funding to help run them would evaporate under the GOP plan. One of the benefits of the exchanges is the ability it gives us to shop online for coverage and compare features and costs of one plan versus another. Health insurance agents and brokers would probably love to see those features disappear, but their gain would be our loss.

To increase competition, the GOP lawmakers say they would allow insurers to sell coverage across state lines. They made no mention of the fact that health insurance is regulated largely at the state level and that federal law doesn't bar insurers from crossing state lines today.

The Medicaid expansion under Obamacare would also go away, as would the federal subsidies available to help low-income families and individuals pay their premiums and cover some of their out-of-pocket expenses. Under the GOP plan, the states would receive block grants from the federal government to help finance their Medicaid programs, meaning the feds would have far less say as to how the states provide coverage to the poor.

The GOP plan would also provide aid only to people making up to three times the federal poverty level--and in the form of refundable tax credits--as opposed to four times the poverty level via federal subsidies under Obamacare.

As a result, many--probably millions--of low- to moderate-income people who were able to buy coverage as a result of Obamacare would once again find the cost of health insurance prohibitively expensive.

Many folks in their 40s, 50s and early 60s would also be dumped back into the ranks of the uninsured or underinsured. Against the wishes of insurance industry lobbyists, Congress restricted insurers' ability to charge older folks more than three times as much as younger people for the exact same coverage when it passed Obamacare. The industry wanted to be able to charge them at least five times as much. The GOP plan would grant that wish.

It's important to note that the Patient CARE Act is not really a piece of legislation. If it were a bill instead of just a "vision," as Burr, Hatch and Upton refer to their ideas, the Congressional Budget Office would have to assess the effect it would have on the budget and our health care system. Researchers for AIS’s Directory of Health Plans find that approximately 27 million covered lives were added last year, a level of increase that hasn’t been seen before in 12 years of tracking.

In-house researchers for Atlantic Information Services, Inc. (AIS), have found approximately 27 million more covered lives across U.S. health plans compared with last year, an increase of more than 10%. Their findings will be published in the upcoming AIS’s Directory of Health Plans: 2015, a data resource which will be available late this month.

Over the Directory’s 12 years of publication, “We have never seen this level of increase before,” says Susan Namovicz-Peat, director of AIS’s directories and databases division. “Typically, we see less than a 3% increase.”

AIS’s Directory of Health Plans: 2015 benchmarks several significant trends in health insurance, including the following:· Health plans attributed more than 6.5 million new members to the first year of public exchanges.
· The individual mandate netted health plans almost 5 million new individual members — a 44% increase from a year ago.
· Medicaid expansion efforts at the state level have resulted in a 30% increase in the number of Medicaid-covered individuals over last year.
· SCHIP enrollment, meanwhile, is down, as many states shifted children into Medicaid plans covering the entire family.
· As of this year, 50% of all health plans are provider-sponsored, and they cover 11% of all members, nationally — almost 33 million people.
· Medicare Advantage (Part C) enrollment is up 10%.

A survey of IT professionals in Fortune 1000 companies released today reports the majority of companies still running Microsoft Windows Server 2003 will not finish their migrations in time for end of support (EOS) on July 14, 2015, despite mandated compliance requirements, such as those governing pharmaceuticals, banking, insurance, as well as any company that processes credit card transactions. Further, with only six months until support ends, the majority of respondents have significant numbers of machines still running WS2003 and almost half of the organizations surveyed still are not aware of, or prepared to deal with, the impending challenge.

The "2014 State of Readiness for Windows Server 2003 EOS Survey" is AppZero's second annual study on how IT professionals are tackling the challenge of Windows Server 2003 end of support. AppZero surveyed close to 500 Fortune 1000 IT professionals throughout 2014 to assess how serious this event is to enterprise IT, determine what preparations are underway and what progress has been made in the year since the first survey findings were published.

The 2014 State of Readiness for Windows Server 2003 EOS Survey reveals that, among those surveyed:·     Almost half (47%) are not aware of the EOS date or have no plans (yet) for remediation
·     Only 21% of respondents have a remediation plan in place
·     Security compliance and vulnerability management remains the largest concern (>50%)
·     Fully one quarter (25%) of respondents still have more than 500+ WS2003 machines

When asked which applications they were most worried about, financial applications topped the list at 33%, the same as when the survey was conducted in 2013. ERP applications (21%) took over the second spot from last year’s second place finisher CRM (20%).

When asked what method of moving to a more modern operating environment was planned, most respondents said they would reinstall applications (32%) or use the Microsoft upgrade path (37%), which recommends uninstalling critical applications, upgrading the machines’ operating system and then reinstalling the applications.

"These two approaches are essentially the same and provide great insight into why most companies will not be finished with their migrations by Windows Server 2003 end of support,” said AppZero CEO Greg O'Connor.

"Remediation is not a question of 'if' but when. The longer it takes, the more the organization will pay due to increased downtimes, failed compliance audits and costs associated with purchasing a Custom Support Agreement from Microsoft. The on-going costs of continuing to run your business on WS 2003 are extremely high and only getting higher as time goes on," O'Connor said. "Don’t delay, start now.”

End of Support for Windows Server 2003 occurs in July 2015. After this date, if a security vulnerability is discovered, Microsoft will no longer patch or address security defects or operational issues. Regulations such as Sarbanes-Oxley (SOX), Payment Card Industry (PCI DSS), Health Insurance Portability and Accountability Act (HIPAA) and Dodd-Frank require certain industries to run on supported platforms.
