Hixny Launches New Patient Portal Serving Capital District, Northern New York and Mohawk Valley

The Director of the New York State Department’s Office of Quality & Patient Safety Patrick Roohan joined Ellis Medicine and Hixny, which operates the health information exchange serving the Capital District, Northern New York and the Mohawk Valley, to share an update on the state of health information exchange in the region. They also unveiled the new Hixny patient portal, which is being rolled out at six regional hospitals, including Ellis Hospital, which allows patients to safely and securely access their complete electronic medical records. This portal empowers patients to safely and securely view, download and transmit their electronic medical records.

Earlier this year, the New York State Legislature voted to support New York’s electronic health records system, the Statewide Health Information Network of New York (SHIN-NY). This innovative, public network will enable doctors and patients to securely access their electronic health records, no matter where they live or work in the state, to improve the quality of care and to help reduce wasteful health care spending.

The Director of the New York State Department’s Office of Quality & Patient Safety highlighted the progress made so far on the implementation of the SHIN-NY, and praised the new Hixny patient portal, which will make it faster and easier for New Yorkers to communicate about their health information with their doctors.

“Hixny and the Statewide Health Information Network of New York are essential to improving the delivery of health care in the Empire State,” said Acting New York State Health Commissioner Dr. Howard Zucker. “By giving doctors and patients the ability to access critical health data when they need it, these networks will help reduce health care costs, and more importantly, save lives.”

Hixny is a Qualified Entity which operates the health information exchange serving more than 1.7 million patients in the Capital District, Northern New York and the Mohawk Valley. Hixny recently rolled out a patient portal with six regional hospitals which allows patients to safely and securely view, download and transmit their electronic medical records.

“We are proud to have established the first community-wide patient portal in New York State and we are excited for its ability to enhance care coordination for the 1.7 million patients in our region,” said Mark McKinney, CEO of Hixny. “This community-wide portal is the latest step on behalf of our unique regional collaborative to support the triple aim of enhanced patient experience, improving population health and reducing healthcare costs.”

“By accessing data through Hixny, Ellis physicians have been able to better understand their patients’ complete medical history, helping them provide better care more quickly,” said Paul Milton, Executive Vice President and COO of Ellis Medicine. “The patient portal allows patients to take a more proactive role in their healthcare, leading to ultimately a safer, better experience. In addition to Ellis Medicine five other regional hospitals are utilizing the Hixny Secure Patient Portal including: Adirondack Health, The University of Vermont Health Network – Champlain Valley Physicians Hospital, Columbia Memorial Hospital, Nathan Littauer Hospital and Saratoga Hospital. When patients are discharged from these hospitals, healthcare professionals create an account which allows the patient to securely access their electronic community health records.

Previously, patient health data on the Hixny network has been accessible only to healthcare providers within the Capital District, Northern New York and Mohawk Valley.

However, with patients’ consent, the SHIN-NY will expand Hixny’s reach, giving healthcare providers instant access to critically important patient health information, no matter where they are located in the state. For example, if a patient from Schenectady needed care while visiting New York City, especially in a medical emergency, the SHIN-NY would allow the treating physician instant access to the information they need to provide effective treatment.

The SHIN-NY also ensures that people, who regularly see doctors in other regions, like the Hudson Valley, can have their health records accessed seamlessly.

In addition to improving the quality of care and improving patient safety, creating a statewide network is expected to save hundreds of millions across the state through reduced re-admissions and eliminating redundant tests. Obama Move On Immigration Prompts Hostility, Gratitude From States

President Barack Obama’s decision to defer the deportation of millions of undocumented immigrants has prompted sharply varying reactions from state leaders, with some vowing to fight the decision and others hailing it as a vindication of their own policies.

Texas and Oklahoma prepared to file lawsuits to overturn Obama’s new policy, while Arizona’s new governor vowed to continue withholding driver’s licenses and in-state tuition from anyone who will benefit from it. Meanwhile, Obama’s decision to offer a deportation reprieve to eligible immigrants echoes policies already in place in California, Connecticut and Rhode Island.

Under the president’s plan, about 4 million unauthorized immigrants who are the parents of U.S. citizens will be eligible to have their deportations deferred and to work legally, as long as they pass background checks and pay taxes. Another one million people will be protected from deportation through other components of the plan.

In addition, the executive action ends the Secure Communities program, under which federal authorities asked local police to detain unauthorized immigrants arrested for traffic violations and other minor offenses. California, Connecticut and almost 300 cities and counties already were limiting or refusing cooperation with federal “detainers,” or requests from immigration authorities to hold someone for possible deportation.

A Department of Homeland Security memo issued on the day Obama laid out his plan acknowledged that “governors, mayors, and state and local law enforcement officials around the country have increasingly refused to cooperate with the program." It said monitoring local jails for deportable criminals "needs a fresh start and a new program."

California has 1.1 million potential beneficiaries of the Obama plan, more than any other state, according to the Migration Policy Institute. According to research by the University of Southern California, 13 percent of the children in California have parents who are unauthorized immigrants and might qualify for the new program. Texas has the second most potential beneficiaries, at 560,000.*Threats of a Lawsuit*While California saw some of its policies enshrined in Obama’s plan, Texas was quick to vow resistance to it. Greg Abbott, the state’s Republican attorney general and governor-elect, pledged to “assert a legal action against President Obama’s unconstitutional abuse of power.” Abbott said Monday a lawsuit from Texas could come in the next two weeks.

“Most everyone agrees that the immigration system in America is broken,” Abbott said. “Similarly, most agree that executive fiat is not the right way to fix it." 

Oklahoma’s attorney general, Scott Pruitt, joined the call for legal action, while Nebraska Attorney General Jon Bruning said his state is considering it. Ken Cuccinelli, a former Virginia attorney general now with the Senate Conservatives Fund, has spoken of organizing opposition by other state officials.

In Arizona, Republican Gov.-elect Doug Ducey has vowed to continue outgoing Republican Gov. Jan Brewer’s policy of prohibiting driver’s licenses for unauthorized immigrants, even for those covered by the Obama plan. He also pledged to enforce a state law prohibiting the state from granting in-state tuition or other benefits to unauthorized immigrants, even if they get work permits under the new federal policy.

Arizona and Nebraska are the only states that do not offer driver’s licenses to unauthorized immigrants who qualify for Deferred Action for Childhood Arrivals (DACA), the existing federal program that allows young people whose parents brought them to the U.S. illegally to remain in the U.S. temporarily as long as they meet age, education and residency requirements.

But it is unclear how much longer Arizona can pursue its driver’s license policy. A federal appeals court on Monday rejected a state request for a rehearing on an order to stop the policy issued in July.*Changing Policies?*A recent report by the Pew Charitable Trusts (Pew funds Stateline) noted that states and localities can play an important part in implementing legalization programs and integrating newly legalized immigrants into their communities. Illinois and Maryland are two states that have pursued immigrant-friendly policies in recent years—but the ascension of Republican governors after years of Democratic rule may prompt them to change course.

Illinois currently ignores immigration status in handing out driver’s licenses, and it has plans to launch an Obamacare-style health insurance exchange open to unauthorized immigrants. Amalia Rioja, an aide to outgoing Democratic Gov. Pat Quinn, said the office is making final preparations for the health care exchange, but no one knows what will happen when Republican Gov.-elect Bruce Rauner takes over in January. Rauner’s transition team has declined to say what he will do about the exchange.

“A lot of what I’m doing could be irrelevant,” Rioja acknowledged.

In Maryland, outgoing Democratic Gov. Martin O’Malley called for fewer deportations and signed a law allowing unauthorized immigrants to get in-state tuition if they attended local high schools and their parents had paid income taxes. It is unclear whether Republican Gov.-elect Larry Hogan will continue those policies.During the campaign, Hogan criticized both Republicans and Democrats in a televised debate on immigration. “I can understand why we want to be welcoming to people who want to come to this country, but we’re also a nation of laws,” Hogan said, blaming Obama and members of Congress from both parties for failing to overhaul immigration. Oracle, Oregon tussle over whether federal court should hear Cover Oregon case

Oracle and the state of Oregon tussled in a U.S. district court last week over which legal venue will hear their dueling lawsuits related to the vendor’s work on the state’s failed health insurance exchange website, Cover Oregon.

Oregon filed suit against Oracle in state court and then Oracle filed suit against Oregon in federal court. In the hearing last Friday, U.S. District Court for the District of Oregon Judge Anna Brown rejected Oracle’s motion to consolidate the cases into one lawsuit to be tried in federal court, citing procedural issues, according to court documents.

The state case didn’t include allegations of copyright infringement, she said. Oracle has since brought up the issue of copyright in the state case, but raised this issue after it requested that the legal proceedings be moved to federal court. The company will refile its motion to consolidate the cases in federal court, according to a report by the The Oregonian newspaper.

The Western world's first gene therapy drug is set to go on sale in Germany with a 1.1 million euro ($1.4 million) price tag, a new record for a medicine to treat a rare disease.

The sky-high cost of Glybera, from Dutch biotech firm UniQure and its unlisted Italian marketing partner Chiesi, shows how single curative therapies to fix faulty genes may upend the conventional pharmaceutical business model.

After a quarter century of experiments and several setbacks, gene therapy is finally throwing a life-line to patients by inserting corrective genes into malfunctioning cells -- but paying for it poses a challenge.

The new drug fights an ultra-rare genetic disease called lipoprotein lipase deficiency that clogs the blood with fat. The medicine was approved in Europe two years ago but its launch was delayed to allow for the collection of six-year follow-up data on its benefits.

*Pricing Filed With German Agency*

Now Chiesi has filed a pricing dossier with Germany's Federal Joint Committee, which will issue an assessment of the drug's benefits by the end of April 2015. The company is seeking a retail price of 53,000 euros per vial, or 43,870 euros ex-factory.

That equates to 1.11 million euros for an typical LPLD patient, averaging 62.5 kg in clinical trials, who will need 42 injections from 21 vials. This price will be subject to a standard 7 percent discount under Germany's drug pricing system. Under German rules, the launch price for a new drug is valid for the first 12 months.

A Chiesi spokeswoman confirmed the launch price, in response to inquiries from Reuters, prompted by information from health insurance sources. She added that a final figure would be set after the G-BA gives its verdict and negotiations are held with statutory health insurance funds. "First commercial treatments are expected in the first half 2015," she said.

UniQure, which will get a net royalty of between 23 and 30 percent on sales, said EU pricing was a matter for its Italian partner, although the Dutch firm does plan to discuss Glybera pricing during an investor meeting in New York on Dec. 1.

*Benchmark Product*

With only 150 to 200 patients likely to be eligible for Glybera across Europe, the impact on healthcare budgets will be small, even at a very high price -- but this case will be watched closely as a benchmark for future gene therapies.

UniQure also has plans to seek approval for Glybera in the United States, which it hopes to get in 2018. Although there is already a gene therapy for cancer on the market in China that drug has not been rolled out to other countries, making Glybera a first for the Western world.

Proponents of the gene-fixing technology insist it stacks up as a cost-effective treatment, despite the high cost, since it could permanently cure many patients.

In the case of Glybera, Chiesi said the annualized cost was no more than that charged for some expensive enzyme replacement therapies used in other rare diseases, taking into account the drug's proven benefits of at least six years.

*How the Drug Works*

The drug consists of a harmless modified virus that carries a corrective gene into the body's cells. UniQure is also working on gene treatments for hemophilia and has an early-stage project in heart failure, which would take gene therapy beyond the rare disease space.

Assuming trials are successful, analysts expect gene medicines treating more common conditions to cost less, since manufacturers should be able to recoup their research and development investment from a larger patient group.

Rivals in the gene therapy market include privately owned Spark Therapeutics, which has an eye drug in late-stage clinical tests, and Bluebird Bio, which is working on drugs for neurological and blood disorders.

Bluebird Bio and UniQure both staged successful floats on the Nasdaq market in the past 18 months, reflecting growing investor interest in the field.

HHS To Push Obamacare On 'Black Friday' The federal government is teaming up with health care groups to promote Obamacare on Black Friday, according to an announcement from the Department of Health and Human Services.

The department and popular websites including The Knot, The Nest and The Bump will promote the controversial health care website to encourage Americans to sign up for health insurance while millions of Americans spend the weekend looking for deals.

The websites will be feature Obamacare-friendly articles such as "A Message from Michelle Obama,""Three Reasons to #GetCovered" and "The #1 Thing to Do on Cyber Monday to Stay Healthy."

“You’ll find a range of insurance choices, lined up side-by-side and explained in easy-to-understand language,” Michelle Obama writes in an article for expecting mothers.

Westfield Shopping Centers and the National Community Pharmacists Association are also joining the promotion scheme.

The Obama administration will promote health insurance coverage at shopping malls starting on Black Friday and continuing through the busiest shopping days of the holiday season, officials announced Wednesday. They said more than 462,000 people selected a private insurance plan in the first week of 2015 enrollment through the online marketplace HealthCare.gov.

The government's enrollment push with Westfield Shopping Centers will involve setting up outreach tables at malls in Florida, Illinois, New Jersey, Connecticut, Maryland, New York and Washington state. Westfield will post information about enrollment services on its website.

*Report Card on First Week*

The administration released what it called a snapshot of signups for the first week of the enrollment period, which started Nov. 15. U.S. Department of Health and Human Services Secretary Sylvia Burwell said 462,125 people chose a health plan in the 37 states using the federal website.

Of those, 48 percent are new customers, including enrollees in Oregon and Nevada, which turned over their troubled insurance markets to the federal government. The figures don't include states running their own insurance markets. The numbers represent only the choice of a plan, and not whether consumers paid their first month's premium -- a requirement for coverage to start.

"We're off to a solid start but we've got a lot of work every day between now and Feb. 15," the last day of the enrollment period, Burwell said in a conference call with reporters. About 1 million people phoned the enrollment site's help line, she said, and roughly an additional 100,000 callers chose to speak with a Spanish-speaking representative. Burwell said the administration is sticking with its previously announced goal of signing up 9.1 million consumers for coverage in 2015.

Unlike last year, the website suffered no outages in the first week, officials said, and it's ready to handle 250,000 users at a time during anticipated surges around deadlines. Consumers must sign up by Dec. 15 for coverage to start on Jan. 1.

*Partnerships With Two Groups*

The figures announced Wednesday don't include dental plans, Burwell stressed. Last week, the administration acknowledged it had been over-reporting the number of enrollees by double-counting about 400,000 who had both medical and dental plans. Burwell said she has directed her staff to find out how the double-counting happened.

Burwell promised a weekly update on enrollment along with more thorough monthly reports that will include what's happening in state-based markets.

Along with the shopping mall campaign, HHS announced marketing partnerships with the National Community Pharmacists Association and the XO Group, a company that runs websites targeting brides, new mothers and homeowners. The pharmacists group will get enrollment information to its members and pharmacy customers, officials said. The XO Group will post blog content on its sites.

ProHEALTH Care Associates, LLP now accepts Empire BlueCross BlueShield insurance plans across its entire network of physicians.

ProHEALTH Care Associates, LLP (ProHEALTH) and its affiliated physicians, has agreed to a new multi-year agreement with Empire BlueCross BlueShield to join Empire's Pathway and Pathway Enhanced Networks. These networks serve Empire members who purchase individual insurance plans on the New York State of Health marketplace, or Exchange, and for Empire small groups and individual members who purchase health insurance plans off of the Exchange.

Empire's Pathway Network is comprised of nearly 49,000 physicians and 83 hospitals that are considered "in-network" for Exchange or Affordable Care Act compliant individual plans. The Pathway networks are designed to focus on primary care physicians who are committed to transforming their practices to a patient-centered model of care to help members achieve a higher-level of care in an efficient and cost-effective way.

As part of the new agreement, Empire Exchange beneficiaries can now access ProHEALTH’s 500 plus physicians, ProHEALTH’s free standing Ambulatory Surgery Center, Diagnostic Imaging Centers, and multiple Urgent Care Centers that were previously considered to be out-of-network. With over 300 medical practices located throughout Manhattan, Brooklyn, Queens, Bronx and Long Island, ProHEALTH brings services directly to where patients live and work. Patients also benefit from the seamless integration of their electronic medical record throughout the ProHEALTH network.
